Does this product allow for database lookup functionality on the pdf form?

Quick PDF Library allows you to find all form fields in a PDF and then get the values for those forms. All of the form field functionality is listed here:

I'm not entirely sure what you mean by real-time database lookups in relation to PDF forms, but if you mean does Quick PDF Library allow you to retrieve values from form fields in a PDF programmatically, then yes, it does.
